Our patient had his Lower left jaw reconstructed due to cancer. He has implants placed in the #19,20 spots. He would like us to send a pre-authorization to his medical insurance for implant codes 6057 and 6061. What ICD-10-CM diagnosis codes do I use and are there any modifiers I need to place on the medical claim?
